Van Dread; a cover and blurb I stumbled across.
Here is how the writer describes it.
You know all those investigators from your CoC games that went insane and were commited or imprisoned? Someone's putting these guys back on the streets, gives them weapons and a mission. A second chance to protect the world from supernatural evil and the minions it employs. And you get to play them!
There's a sanity mechanic. All characters have a derangement (they have been diagnosed since they are in fact incarcerated), but they can choose if they are truly insane or not(since a character could have been incarcerated because he was talking about monsters that he knows exist, yet everyone else doesn't). Medication helps keep the effects under check.
I'm thinking that every character is a ticking bomb. If they get stressed out too much, they snap and either kill themselves or go on a murder spree(according to their diagnosed derangement. Effects may vary).
Characters are essentially paranormal investigators, only with a more hands-on approach. They use guns, artifacts(enchanted items) and spells(heavily based on components and ritualism) to achieve their goals.
Main inspirations are John Constantine comics and a bit of BPRD. In essence, I want to make a game that's scary. Sure you can shoot shit and act tough, but you're scared shitless while doing it.

Hunter the Vigil would be a good fit as would this (which I happened to pick up yesterday.)
But it is a cool concept. The player characters are the folks that went off the deep end, it continues on after Lovecraft's stories end.
